How to create automated intake form

I see that I can create an edited optiom intake form with TypeForm. Is there a way to include the form link to a specific service so when the client receives the automated text, they are prompted to fill out the form?

Yes, if you go to your Appointments tab and then scroll down in the left side menu you will see SETTINGS ---> COMMUNICATIONS.   

On that page, scroll down to FORMS. ---> Add a Form.  

All your templates should pop up and you can assign the form to specific services.  That way when clients book the form will auto-send to the client.  You can also set it to send every time or just once a year or just the first time, etc.  I have different intake form for different treatments and this has been super helpful.
