How do I add a form so that I can export a CSV of customers who have purchased a certain item?

I am offering a combination of classes products and consultations through my site and being able to set up forms that link to individual items would be incredibly helpful. Is there a way to do this?

Thanks for reaching out. I'm a little confused with your post. Are you asking if you can export a csv for your own records to keep track of who purchased what item? Or are you asking if it's possible to add a form to a product with specific details/information you need from the customer for the class?
