Gift card page not loading properly

Tried sending this to Weebly support but no word yet. I just implemented gift cards to my site and when I tested the purchase process the required text boxes don't actually appear (recipient name and recipient email) on the page so I can't start the process of buying a card. This is on laptop.

On mobile (Android) none of the info that needs to be filled out even appears. 

Any advice? Thanks in advance.

Hey Adam, I haven't gotten any help yet and just looking for an update. The main help center says there's a live chat option at the top of the page but I don't see it there. Do you know if this has been an issue for other users?

Sorry about that, @BeeKingdomGlass. It seems like it's a style issue - possibly with that theme - it's hiding the display of the fields. You can try switching to another theme temporarily to see if it fixes it, otherwise this should do it on the live site if you add it to Settings > SEO > Footer Code:
