Add Form to weebly site

Dear weebly,

How can i add survey Form to weebly? And after embed code can user only see my domain (not launchcloud)? 

I have code: embed HTML of this servey:

Hey @bvotgroup! Add the "head" portion to Settings > SEO > Header Code, and the other two bits of code to an Embed Code element, then publish your site.
