Editable Dispatch or Delivery notes

I really like using Weebly, but I think the packing slip / delivery note looks awful and should be editable. I would like to be able to add text such as "thanks for your order, if you have any queries etc.." and also add my company logo.

The delivery note is often the first thing that the customer sees when opening the package, so it is an important opportunity to engage with the customer, and should look better than the current weebly one.
