Contact Us form submission...when I export some have info missing......

We created a Contact Us Authorization Form to be completed   

Many of the exported responses have a 1 vs the selected response (ie one question is "how did you get your TBI", they can select the answer(s) that applies) or a response is missing (even though it is required).  

Only in areas where they can type in a comment do you see the answer. Rest show a 1 when they can click on a "button" or "checkmark"
