Issues with Customer Refunds

Has anyone had an issue whereby they refunded a customer, but the customer had closed their account (the refund took place several months later) and the bank rejected the refund, but Square is still saying that the refund is processed on their end?  Square is telling me to have the customer file a dispute at their bank, but the customer said the bank is telling her she is unable to file a dispute because they show that the refund was rejected.  The customer is demanding I cut her a check, but Square already took the money out of my account for the refund!  Has anyone had this issue?  How did you resolve?
