Square Appointments: Ability to upload consent forms

I own a small Permanent Makeup Studio and It would be great to be able to either upload my own consent forms or be able to send a link to consent forms in email or text when scheduling appointments.

 That way they are kept digitally and filled out before their appointment.

Has this been addressed or is there more info? This is standard offering for Acuity, Vagaro, all the companies the beauty industry uses. We have intake forms we need this integrated. After 6 years of waiting I am ready to move services to get this basic function.

Not sure why but when I try to follow your here link all I get is:

I was just trying to find out more information about contracts with people setting up an appointment.  I wanted to start having candle making classes, but wanted to attach a waiver that students would have to sign before the class plus have a non refundable deposit.  A local shop near me use to do Hand Dipping but people would sign a waiver then try to sue that business because the wax was too hot for their child.  Even though the wax was cooler than the temperature listed on the waiver that the wax could reach.  Since people would be dealing and handling wax which reaches 170 to 180 degrees f, I would need this waiver attached and signed before paying for a class.

So right now I am in the research stages to get all my i's dotted and t's crossed before doing these classes.

This is needed for intake for clients that are incoming for skincare and massage in our case.  We need to take a medical history and other pertinent information beforehand to assess how the appointment may move forward and assess any possible contraindications.
