Is there a way for me to add a medical history form that the client needs to fill out at the time they are booking their appointment? I do not want to send it to them after they have booked it, I would like them to fill out this form in order to make a booking. In context, I am a tattoo studio and would like to know the type of tattoo they would like before they have booked their appt.

Square does not have the feature you're looking for however, there is a possible 3rd party option - see this info.
