I would love to see an Intake Form feature with Square Appointments. A customized template that is required for clients to complete as they book an appointment would be incredibly useful.

 

This would help us gather more information from clients before they come in. This would also save my team time on the back end. Many businesses currently have to email out our consent forms to each new client and make sure it is complete before their appointment. It would be easier if the client had to complete the form as part of the online booking process.

 

I know Contracts can be sent to clients after the fact, but they are not quite the same as intake forms during the online booking process.

Agreed! I added custom wording to the appointment notification email, but clients rarely check their email. I always have to send them a text asking them to complete their intake forms. I'm solo and this feature would help so much!

This would be fantastic for new clients. I have to open multiple windows to transfer, copy and paste client info into appointments from square online form. An intake form that plugs info directly into the system would be fantastic.

We need this for medical conditions and allergies.  Right now I use a typeform post booking but I can't import info into customer profile thru API unless I have our nurses manually enter it into the customer custom data field.  If we could have this worked into the booking flow it would be great to have this as next step after entering card info and confirming their appointment time.

 

Then the information can be stored on customer profile for easy access at each appointment to make sure our nurses are not risking anyone's health.  We do this all manually right now and would be nice to free up the workload on our nurses and IT dept.

Yes! So archaic, I have people fill out a consent form to groom their dog upon drop off. I should be be able to have them fill out an electronic form upon booking sent from the Square system via text /email like a LOT of other business's have the capability to do (as I've had first hand experience in filling out myself). It should then populate a note for their appointment if form was filled out prior or if it was still needing to be filled out.

Also love this! I currently have online booking ONLY for existing clients with a password protected page on my website and am old school with a new client form to fill out when they arrive to a consultation or their first appointment. Would like to have something like FORMS that we can customize way more than Contracts, which just doesn't cut it for what we need it for.

Please please please pretty please do this for me! 

 

This is called waivers on vagaro and as custamizable on the booking flow. 

 

I would also ad "How did you hear about us" - Ive mentioned elsewhere, Ill gladly pay $20 extra monthly for this. 

Yes we need intake forms added asap.Even insulation forms . @Square hurry up...

So I have an idea - Money talks.   I would, I sincerely mean this, pay $20 extra for the waiver feature if it was done at least as good as their competitor Vagaro has done. 

 

Everyone else list how much this intake forms feature is worth to you, and be honest. I know nobody on here is paying $80 a month for it. 

(I would also pay an extra $15 to be able to customize the appointment confirmation text, but that is for another forum thread)
