Can I customize the information that's collected in the Online checkout process?

Trying to add an online check out link for patients to pay their balances on website through Square. Is there a way to customize what information is submitted on the form associated with the online check out link? Was hoping to find a way to add a phone number or date of birth so we could better identify who is making the payment.

I know, it seems like yesterday but it has now been 4 years. David Bowie and Prince were still alive... Flint Michigan was still drinking contaminated water... and this form feature was requested.
