Can I set up a custom payment form?

Hi, I'm hoping someone can help me with this.

 

We're a small ambulance service and are trying to set up to have patients pay their bills online. We can't really afford to upgrade on our Wordpress site to use WP Easy Cart or anything like that at the moment. Here's what we'd like to do:

 

We're hoping to set up a custom form or something run by Square where people can put in their information and a custom payment amount and then submit it to be applied to their account. We're okay with it being hosted directly on Square where we just link to redirect people to a special Square page to do this. We just don't know how to do it. Does anybody have any advice on the best way to go about this? Any suggestions for any easier alternatives?

The best suggestion I have for you would be to check out Jotforms and see if this is an integration you could use with your WordPress site to solve the issue.

If you're taking payments for Appointments online, you might want to consider enabling customer prepayment for Square Appointments following this guide
