"Where did you hear about us?" option on order submission

Really need functionality to add a question in the checkout process.

Thanks for posting @soilwater. It looks like this was suggested in the past on our Vote on Feature board. The search function is not working on our forum at the moment. I tried to find an active requests, but was not able to find anything due to the Lithium error.
