Opt-In Forms Don't work on my page

Please help! I have an ad running well on FB however, I'm unable to lock the screen of my pop-up and I've not received ANY emails even though I've had over 60 visits in 24 hours. Please Please help—I do NOT want to stop my campaign. It took hours to launch bc of COVID.
