Additional Info questionnaire and Credit Card Authorization Form requested by Square

Square has requested additional information regarding our business and transaction types. One prompt in the questionnaire asked us to provide signed credit card authorization forms for two separate customers/transactions. We do not process the payments directly for the customer each time, and for these two specific clients they provided their credit card/ACH info and submitted the payment directly via their invoice. Is this something that would be required each time?
