When employee permissions are checked for the employee to access sales transactions in the retail app on the ipad, Square only allows 4 transactions visible and no access to return items or previous transactions.

Is there any chance that someone signed in to a different location on this device? As long as you've enabled the "Issue Refunds" permission setting for the correct set of employees, they should be able to issue refunds from the app.
I'm not able to access and troubleshoot account settings from our online forums, but I can see that you have a case open with our Retail Support Team — they'll be able to take a look behind the scenes and make sure this gets sorted out ASAP.
