Need help setting up shipping for print-to-order merch from a third-party supplier

I've set up a shipping profile with flat rates for each item (first and subsequent in each class of merchandise).

Now, I need to ensure that each rate attributes to the correct item when customers add items to their cart.

Does anyone know how to configure this?
