How to create shipping slip with my company information on it

Looking to be able to print a shipping slip with the customers' order and my company information (logo, address, contact info).

The print order option under 'orders' does not look to have an option to add my Company header and contact information.

Please send any recommendations if you have been successful with this.

Have you tried checking under > Settings > Shipping from the Online Store tab on Dashboard?

On my test account that's where my name and address shows so I'm not sure if you already have your information in there or not.
