customer profile missing

I have been having trouble with a customer's profile.  She's been a client for almost a year now and for some reason her profile is now missing.  I am the only one that manages customer profiles so it wasn't accidentally deleted.  I noticed at her last appointment that both her email and phone number were hidden and she wasn't receiving confirmation texts or emails like I have set up in the system.  Any insight is appreciated.
